1. You need to download, fill out and bring (or e-mail) these New Gyn Forms before the doctor sees you.
2. Even if you are on time for your appt. but these forms are not filled out, you might not be seen.

We really need all this information from NEW patients. It's either required by law, is for billing, or it's your medical history that the provider has to review before you can be seen.

Note: We accept most PPO insurance plans but we do not accept HMO or Medi-Cal insurance.
